Warren County food service inspections listed
Both the City of Warren and the Pennsylvania Department of Agriculture have released the results of recent food service inspections.
Inspections have been completed and the following establishments have valid licenses until May 31, 2014:
Wellness Cafe & Coffee Shop, 340 Pennsylvania Ave. W., one critical violation – cutting board needs replaced or properly cleaned.
Bi-Lo, 4 Market St., two critical violations – staff hand washing station not properly used, meat and cheese not properly marked or disposed of when outdated; one non-critical violation – food stored in boxes on floor in walk-in.
Shur-Fine, 1613 Pennsylvania Ave. E., two critical violations – chicken not at proper hot holding temperature, outdated lunch meat in deli case and some not properly labeled.
Dollar General, 40 Market Plaza, no violations.
Trinity Memorial Episcopal Church, 444 Pennsylvania Ave. W., no violations.
Shell Service Center, 1701 Pennsylvania Ave. W., one non-critical violation – thermometers not provided in all appliances.
The following establishments have valid licenses until June 30, 2014:
Fat Daddy’s, 601 Pennsylvania Ave. E. (formerly V&J Pizza), one non-critical violation – thermometers not provided in all refrigerators.
Christie’s Downtown Pub & Grill, 229 1/2 Pennsylvania Ave. W (formerly Busy Bee), no violations.
Snuffy’s Cafe, 208 Hickory St., no violations.
Bill & Debbie’s Ice Cream, 37251 Rt. 6, no violations.
Elks Club, Hickory St. and Fourth Ave., no violations.
Family Dollar, 1036 Pennsylvania Ave. W., no violations.
5th Ave. Cake Works, 211 1/2 Fifth Ave., one critical violation – staff not Servsafe certified.
Six-month inspections were completed at the following establishments:
Little Caesars, 2 Market St., one non-critical violation – all staff not wearing hair restraints in prep area.
Counts, 910 Pennsylvania Ave. W., no violations.
McDonalds, 11 Market St., one non-critical violation – thermometers not provided in all freezers.
Plaza Restaurant, 328 Pennsylvania Ave. W., one critical violation – BBQ pork not at proper hot holding temperature (rest of steam table at proper temp), one non-critical violation – ice cream stored on floor in walk-in freezer (recently received order and delivery.)
The Department of Agriculture released the results of the following inspections in Warren County conducted in May and June.
Bear Path Inc – DBA: Tippy Canoe, 99 Hill Dr., Tidioute, two violations: Food facility does not have available sanitizer strips or test kit to determine appropriate sanitizer concentration of quat ammonia, used in the bar three basin sink; build-up of slime in the soda gun holster in the bar area, clean immediately.
Big Lots, 4000 Market St., two violations – small amount of bird seed spillage on the retail shelf near the animal food, multiple ceiling tiles are missing throughout the sales floor area and need replaced. PIC stated it is due to an air conditioner leak throughout the entire mall and the tiles are removed to prevent water saturation. Tiles will be replaced once the owner of the mall has the air conditioning unit repaired.
Blue Jay Inn, 4114 Blue Jay Creek Rd., Sheffield, two violations – heavy dust buildup under the counters at the bar area over the three basin sink, clean immediately. Debris/litter located under the three basin sink in the bar area, and under the cook line in the food prep area; Heavy dust build-up under the counters at the bar area over the three basin sink, clean immediately, Debris/litter located under the three basin sink in the bar area, and under the cook line in the food prep area.
Blueberry Hill Golf Dev, 6528 Cable Hollow Rd., Russell, two violations – heavy dust build-up under the counters at the bar area over the three basin sink, clean immediately. Chase Auction, 5555 Rt. 957, Russell, one violation – heavy dust build-up under the counters at the bar area over the three basin sink, clean immediately. Debris / litter located under the three basin sink in the bar area, and under the cook line in the food prep area.
Cherry Grove VFD, 6045 Cherry Grove Rd., Clarendon, no violations.
Dairy Delite, 1 Kinzua Rd., Clarendon, two violations – the door track to the reach-in freezer next to the hand wash sink has an accumulation of debris. Open containers of ice cream are stored in this unit. Ice cream needs to be covered and the door tracks need to be cleaned immediately, as it could possibly lead to food contamination.

Eisenhower Middle/High School, 3700 Rt. 957, Russell, two violations – door seal for the walk-in cooler has been patched with duct tape. The duct tape is ripped/frayed, and not easily cleanable. When facility begins construction for the new kitchen, make sure the door seal is repaired/replaced, and duct tape removed; Still a heavy accumulation of dust on the back fins of the cooling unit in the walk in cooler.
Fox Mart, 1 Cable Hollow Rd., Russell, no violations.
J’Z Texas Hot, 3 Railroad St., Warren, four violations – commercially processed ready to eat food, a bulk container of ketchup, located in the home style reach-in refrigerator in the storage shack, and held more than 48 hours, is not being marked with the date it was opened; the handwash sink in the food prep area was blocked by the potato slicer and not accessible at all times for employee use. Owner moved the slicer immediately; both tube lights in the mobile unit have broken / cracked tube shields exposing the glass lights, and making them incomplete shield systems. Owner stated he will have these replaced; a Food Employee Certification is not displayed at the facility. Owner stated that he just got re-certified for ServSafe. Owner provided his ServSafe course book, Record/Class #1233555, and date the test was issued of 4/15/13. Owner to mail/fax a copy of the FEC to the Region 1 office in Meadville at the fax number or mailing address provided on the business card provided at the time of the inspection.
Landmark Hotel, 99 Main St., Tidioute, two violations – one stem type thermometer had a crack in the top exposing the read-out needle. Owner discarded the thermometer, a back-up thermometer was available and accurate; heavy build-up of grease and dust on the Ventilation Hood System filters and surrounding framework above the grill line. Owner stated that it will be cleaned soon with the installation of a new fan motor.
Polly’s Shur Save Market, 148 Main St., Tidioute, Sherer brand name reach-in freezer observed to have pitting, flaking, and rust on the interior top wall near the air curtain ventilation; one stem type thermometer indicated 28 degrees F during an ice point calibration, thermometer adjusted to 32 degrees F; there is dust build-up on top of the blades for the fans in the deli food prep areas that need to be cleaned.
Sheffield Area Medical, 511 S. Main St., Sheffield, one violation – there is a build-up of dust on the air vent and ceiling tile nearest kitchen entrance that needs to be cleaned.
Sheffield VFD, 381 S. Main St., Sheffield, no violations.
Shooters BBQ, 9 Liberty St., Russell, no violations.
Tidioute Hotel, 128 Main St., Tidioute, one violation – still some areas of the flooring in the back food prep area that need to be repaired / replaced. Obvious repairs have been made, and is still a work in progress.
Tops Markets, 28 Railroad St., Youngsville, two violations – walk-in freezer cooling unit is leaking condensate onto bags of ice stored below, as discussed food products must be removed from under the cooling unit immediately until repairs can be made. Ice was moved to another location at the time of the inspection; flashing in the walk-in milk cooler is in disrepair and separating from the interior wall. PIC stated a call will be made to have the cooler serviced, there is loose hanging sealant on the interior ceiling of the walk-in freezer and a heavy build-up of frozen condensate on the interior ceiling and cooling unit.
Towne Tavern, Main and Railroad St., Clarendon, four violations – commercially processed ready to eat food, bulk containers of dressings, located in the reach-in cooler nearest the ice maker, and held more than 48 hours, are not being marked with the date it was opened. Correct immediately; stem type thermometer indicated 38 degrees F and was adjusted to 32 degrees F during an ice point calibration; the handwash sink in the dish wash area was blocked by a home style electric sweeper and not easily accessible for employee use. Sweeper was moved during the time of the inspection; lights are not shielded or shatter proof over the grill line area. The cover for the light fixture over the sandwich prep unit has a large hole in the center.
Wendy’s Cafe, 200 Main. St., Russell, four violations – observed wet and soiled wiping cloths in the grill line area, not stored in sanitizer solution; stem type thermometer needle sat on the logo between 0 degrees F and 220 degrees F rather than 32 degrees F when calibrated using the ice point method. Adjusted during the time of the inspection; there is a build-up of residue in the soda dispenser nozzles (Diet Pepsi & Mug Root Beer) and surrounding nozzle connection areas, clean immediately; the filters on the ventilation hood system appear to recently cleaned, but the surrounding framework has a heavy build-up of grease, grease droplets are starting to form over the cooking area. If this is not cleaned soon, it could potentially contaminate food on the grill line. There is a heavy accumulation of grease on the floor underneath the cart located next to the fryer.
